Client Rights & Consumer Information
In accordance with Texas law (Health & Safety Code §181.105), the following information is provided to assist consumers in understanding their rights related to health care records, regulatory contact information, and the complaint process.
Requesting Your Health Care Records
Clients have the right to request access to or copies of their health care records.
How to request records:
- Submit a written request to: intentionallyresilient@gmail.com
- Requests must include:
- Client’s full name
- Date of birth
- Signature authorizing release of records
- Records will be provided within the timeframe required by Texas law.
- Reasonable fees may apply, as permitted by law.

Filing a Consumer Complaint
Consumers may file a complaint regarding services received with the Texas Office of the Attorney General – Consumer Protection Division.
